The following are some examples of possible Antecedents, Behaviors, and Consequences.
Please indicate the number in the columns above and make notes about what you saw.
The ABC Observation Form is helpful in gathering information on a specific behavior for a Functional Behavioral Assessment. Teachers often use a process like this to gain insights on individual children and their class as a whole. Through a process of observation, interpretation and reflection teachers are able to identify patterns and develop strategies.
Please reflect back on/note two different children/behaviors from your observation.
Jot brief notes or key words to remind you of exactly what you observed before the behavior (antecedent) how the behavior was displayed (behavior) and then how the teacher responded (Consequence).
In the column under “Antecedent Events,” think about what happened immediately prior to the child displaying the behavior and write your observation. Jot down brief notes to help you recall what happened.
In the Behavior column, write down what you observed and heard, what did the child do, what did the other children do, what did the teacher do. Jot down notes to help you recall what happened.
In the column under “Consequent Events,” write down what you observed, what did the teacher do, what did the child do, maybe even note what the other children did. Make some brief notes to help you remember what form the consequence took.
